Ottawa Kangaroos Volleyball Club

The Ottawa Fusion Volleyball Club is holding registration for all athletes on Tuesday, October 6, from 6 to 9pm at Sawmill Creek Elementary School (3400 D’Aoust Ave, Gloucester).  At this time, all of the forms included in the registration package must be submitted, along with fee payment.  Athletes will be fitted for their uniforms and will have height measurements and head shots taken for the website.

OFVC Medical Form
OFVC Player Agreement
OFVC Parent Agreement
OFVC Player Profile
OFVC Parent Power Form
Ontario Volleyball Association (OVA) Individual Registration Form.

All the above listed forms, with the exception of the OVA Registration Form can be found in the registration package which will be available via email or for download by Sunday night (check the website or your email).  It is recommended that you download the package and prepare the forms ahead of time.  The package will also be available on registration night for  those who have not already received it.  The OVA Registration Form will be electronically prepared and available for signature at registration.  PLEASE NOTE:  The OVA form requires parent/guardian signature in order for us to process registration. For this reason, a parent/guardian must accompany the athlete to registration as these forms will only be available onsite.  If this is not possible, your form can be made available via email prior to Oct 6.  Please contact Lianne Normand (lianne.normand@rogers.com) if you need a copy ahead of time.

There is no specific time for teams to register.  You may come by at your leisure between 6 and 9pm.  Please plan on 20-30 minutes to complete the process.
